Job summary

ArchWell Health in Tucson, AZ seeks a licensed clinical social worker (LMSW) to join our patient‑centered care team. You will collaborate with primary care to assess social needs and remove barriers to care in a welcoming, community‑based clinic.

Qualifications

• Master's degree in Social Work and LMSW licensure in the practicing state.
• 1+ years of experience in clinical social work.
• Strong problem-solving orientation and adaptive attitude.

Responsibilities

• Develop relationships with primary care teams to conduct social services assessments.
• Plan and coordinate support packages to overcome socioeconomic and medical barriers.
• Navigate managed care plans for community services and programs.
• Caseload management for social and behavioral health to enable self-management.
• Document patient encounters accurately in clinical management systems.
• Identify improvements via feedback and regular surveys.
• Support advance care transitions with members and families.
• Assist members in accessing state prescription programs and other benefits.

Duties/Responsibilities:

• Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each member
