Job summary

ArchWell Health in Florida is seeking a licensed social worker to join our team serving seniors. You will assess needs, plan support packages, and coordinate services across care teams to reduce barriers to care.

The ideal candidate has an MSW or BSW with 1+ year of clinical social work, strong communication, Spanish language skills, and a collaborative, compassionate approach aligned with our mission to deliver accessible, high-quality senior care.

Qualifications

• Fully licensed BSW or MSW in the state of practice.
• Master’s degree in social work is preferred; Bachelor’s is required.
• 1+ years’ experience in clinical social work.
• Problem-solving orientation with a flexible and positive attitude.
• Mission-driven to transform senior care delivery.
• Proficient PC skills.
• Fluency in Spanish or other languages spoken in the communities served.

Responsibilities

• Develop relationships and collaborate with primary care teams to conduct social services assessments.
• Plan, coordinate, manage and implement support packages to help members overcome barriers.
• Navigate managed care plans for community services and programs.
• Case management for social and behavioral care to enable self-management of health and support.
• Document patient encounters accurately in clinical management systems.
• Identify methods to improve ArchWell Health’s approach from feedback and surveys.
• Support advance care transitions with members and families.
• Assist members with access to state-based prescription programs and benefits.

Duties/Responsibilities

• Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each member.
• Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers.
• Navigate managed care plans for community services and programs.
• Case management for social and behavioral care to allow members to self-manage health and social service support.
• Accurate and timely documentation of patient encounters and sessions in all clinical management systems
• Proactively identify methods to improve ArchWell Health's approach based on feedback and regularly conducted surveys.
• Support advance care transitions with members and their families
• Assist members with access to state-based prescription programs and other benefits.

Required Skills/Abilities

• Experience supporting patients with need for social services.
• Experience with screening, assessment, and planning for common social services needs
• Working knowledge of differential diagnosis of common mental health conditions
• Strong interpersonal communication skills with exceptional active listening abilities
• Highly empathetic, non-judgmental, and open-minded
• Experience in a collaborative team environment

Education and Experience

• Fully licensed BSW or MSW in the desired State of practice
• Master's degree in social work is preferred, Bachelor's is required
• 1+ years' experience in clinical social work
• A problem-solving orientation and a flexible and positive attitude
