Job summary

ArchWell Health is seeking a licensed social worker to join our team and coordinate care for senior members within neighborhood clinics. The role focuses on assessing social service needs, planning support packages, and advocating for access to community resources.

You will collaborate with primary care teams, document encounters in management systems, and help members navigate benefits and prescription programs. Fluency in Spanish is a plus.

Responsibilities

  • Develop relationships and collaborate with the primary care team to conduct social services assessments.
  • Plan, coordinate, manage and implement support packages for socioeconomic and medical barriers.
  • Navigate managed care plans for community services and programs.
  • Case management for social and behavioral care to enable self-management of health and social services.
  • Accurate and timely documentation of patient encounters and sessions in clinical systems.
  • Identify opportunities to improve ArchWell Health's approach from member feedback and surveys.
  • Support advance care transitions with members and families.
  • Assist members with access to state-based prescription programs and other benefits.

Duties/Responsibilities:

EducationandExperience:
  • Fully licensed BSW orMSWin the desired State of practice
  • Master’s degree in social work is preferred, Bachelor's is required
