Job summary

ArchWell Health is seeking a licensed LMSW to join our care team in providing social work services to senior members. You will collaborate with primary care, assess needs, and coordinate support packages to address socioeconomic and medical barriers.

Expect to navigate managed care plans, document encounters in clinical systems, and support transitions with families. Bilingual skills and strong empathy are valued in a mission-driven environment focused on accessible, high-quality elder care.

Qualifications

• Fully licensed LMSW in the desired state of practice.
• Master's degree in Social Work.
• 1+ years' experience in clinical social work.
• Problem-solving orientation and flexible, positive attitude.
• Mission-driven and motivated to transform elder care delivery.
• Proficient PC skills.
• Fluency in Spanish or other languages spoken by communities served.

Responsibilities

• Develop relationships and collaborate with primary care team to assess needs.
• Plan, coordinate, manage and implement support packages for socioeconomic and medical barriers.
• Navigate managed care plans for community services.
• Casemanagement for social and behavioral care to enable self-management.
• Accurate and timely documentation in clinical management systems.
• Identify methods to improve ArchWell Health's approach via feedback and surveys.
• Support advance care transitions with members and families.
• Assist members with access to prescription programs and benefits.
