Job summary

Mosaic Contractors in Arizona is seeking a Home Provider to welcome and support an individual with an intellectual disability in your home, helping them build independence and community connections.

You will assist with daily living skills, participate in health appointments, and provide transportation as needed. Training and ongoing support are provided, with a focus on belonging, growth, and shared family-like dynamics.

Qualifications

• Must be at least 21 years of age.
• Arizona resident due to state funding restrictions.
• Own/rent your own home, apartment, condo, or townhouse.
• Reliable transportation, valid driver’s license, and current vehicle insurance.
• Internet access and basic computer skills.
• Ability to complete required screenings, background checks, and training.
• Experience is preferred but not required.

Responsibilities

• Assist with daily living skills such as cooking, budgeting, cleaning, and decision-making.
• Assist with medication administration and medical appointments.
• Help individual build connections and participate in community activities.
• Provide transportation and support with daily activities as needed.
• Maintain documentation and communicate with Mosaic support team.
• Help build confidence, independence, and personal goals.

Average Annual Income

Approximately $42,500
