Job summary

Mosaic in Colorado is seeking dedicated Home Providers to create a meaningful and inclusive environment for individuals with intellectual disabilities. As a Home Provider, you'll help individuals thrive by offering support in daily activities and fostering community connections.

This role requires a commitment to helping individuals feel safe and valued, as well as reliable transportation, a stable home environment, and the ability to complete necessary background checks. Experience is preferred but not required.

Qualifications

• Must be at least 18 years old.
• Current resident of Colorado, own/rent a home.
• Valid driver's license and reliable transportation.
• Internet access and basic computer skills.

Responsibilities

• Support daily living skills such as cooking and budgeting.
• Assist with medication administration and appointments.
• Help build connections and participate in the community.
• Provide transportation and support with daily activities.
• Maintain documentation with the Mosaic support team.

Position Snapshot

Location: Must reside in Colorado (Must own or rent your current home; Limited availability in Aurora; Not Available in all counties)

Supporting: You are welcoming an individual with an intellectual disability to move into your home and live with you full-time as a part of your household and family dynamic

Average Annual Income: Approximately $46,530 (federally tax-free) + Monthly Rent & Household Expenses Contribution

How You'll Make a Difference

Every individual is unique, but your responsibilities may include:

• Supporting daily living skills such as cooking, budgeting, cleaning, and decision‑making.
• Assisting with medication administration and medical appointments.
• Helping someone build connections, participate in their community, and experience a sense of belonging.
• Providing transportation and support with daily activities as needed.
• Maintaining documentation and communication with the Mosaic support team.
• Helping someone build confidence, develop independence, and pursue personal goals.

Depending on the individual's needs, support may also include:

• Assistance with personal care activities
• Behavioral support
• Medical or physical support needs
• Specialized care and monitoring

Requirements

• Must be at least 18 years of age
• You must be a current resident of the state of Colorado (due to state funding restrictions).
• Own/Rent your own home, apartment, condo, or townhouse
• Active homeowner's or renter's insurance
• Reliable transportation, valid driver's license, and current vehicle insurance
• Internet access and basic computer skills
• Ability to complete required screenings, background checks, and training

Experience is preferred but not required.
