Job summary

Mosaic invites you to become a Home Provider in Arizona, welcoming an individual with intellectual disabilities into your home full-time. You’ll help with daily living, transportation, medical appointments, and community participation, supporting their growth and independence.

Applicants should be 21+, reside in AZ, own or rent a home, and have internet access. Training and team support are provided throughout the matching process, with ongoing partnership to ensure successful outcomes.

Qualifications

• Must be at least 21 years of age.
• Arizona resident (state funding restrictions).
• Own or rent your home, apartment, condo, or townhouse.
• Active homeowner's or renter's insurance.
• Reliable transportation, valid driver's license, and current vehicle insurance.
• Internet access and basic computer skills.
• Ability to complete required screenings, background checks, and training.
• Experience is preferred but not required.

Responsibilities

• Supporting daily living skills such as cooking, budgeting, cleaning, and decision‑making.
• Assisting with medication administration and medical appointments.
• Helping someone build connections, participate in their community, and experience a sense of belonging.
• Providing transportation and support with daily activities as needed.
• Maintaining documentation and communication with the Mosaic support team.
• Helping someone build confidence, develop independence, and pursue personal goals.

Position Snapshot

• Supporting: You are welcoming an individual with an intellectual disability to move into your home and live with you full-time as a part of your household and family dynamic
• Average Annual Income: Approximately $42,500 (federally tax-free)
