Job summary

Living Innovations in Warwick, RI is seeking a grounded Shared Living Coordinator to support individuals, families, and contracted home providers. You’ll manage your own caseload, develop person‑centered support plans, oversee health and safety, and advocate for resources to meet needs.

You will visit homes, ensure environments are safe and compliant with state standards, coordinate training for providers, and handle on‑call coverage while maintaining confidential records and billing

Qualifications

• Minimum 1 year of hands-on experience in social services, healthcare, or direct care support.
• Bachelor’s degree preferred but not required; minimum high school diploma or equivalent with relevant experience accepted.
• Valid driver’s license, reliable vehicle, current auto insurance, and the ability to pass standard state background checks.

Responsibilities

• Develop and guide individual support plans and oversee health and safety needs.
• Assist with home transitions and advocate for resources the person needs.
• Visit homes regularly to ensure environments are safe, welcoming, and compliant with state standards.
• Manage caseload, maintain case notes, and ensure billing documentation is accurate and confidential.

You’ll manage your own caseload, keep details in order, and act as a reliable advocate for the people we serve.What You’ll Be Doing Day-to-DayBuild real, trusting relationships. You’ll help develop and guide individual support plans, oversee health and safety needs, assist with home transitions, and advocate for whatever resources the person needs.Partner closely with our contracted providers, ensuring they have the training, certifications, and day-to-day backing required to offer exceptional care.Visit homes regularly to make sure environments are safe, welcoming, and meeting all state quality standards.Be a steady presence during tough moments.
