Job summary

Living Innovations is seeking a grounded Shared Living Coordinator in Warwick, RI. You will work directly with individuals, families, and home providers to ensure safety, resources, and person-centered supports across your caseload.

The role emphasizes building strong relationships, coordinating with providers, ensuring state quality standards, handling emergencies with calm judgment, and maintaining accurate paperwork and billing documentation.

Qualifications

• At least 1 year of hands-on experience in social services, healthcare, or direct care support.
• High School diploma or GED; Bachelor's degree preferred but not required.
• A valid driver's license, reliable vehicle, current auto insurance, and the ability to pass standard state background checks.

Responsibilities

• Build real, trusting relationships. Help develop and guide individual support plans, oversee health and safety needs, assist with home transitions, and advocate for needed resources.
• Partner closely with contracted providers to ensure they have training, certifications, and day-to-day backing to offer excellent care.
• Visit homes regularly to ensure environments are safe, welcoming, and meet state quality standards.
• Be a steady presence during tough moments, share on-call coverage, handle emergencies calmly, and work through complex challenges with good judgment.
• Stay on top of state paperwork, file self-checks, case notes, and billing documentation to keep services compliant and accurate.

Education

High School diploma or GED

Bachelor's degree in social services preferred
