Job summary

Position Overview – Residential Support Coordinator

As a Residential Support Coordinator, you will play a vital role in coordinating services for individuals participating in our residential program. Working closely with individuals, families, Direct Support Professionals, clinical teams, and community partners, you will ensure that each person's services are thoughtfully planned, effectively coordinated, and tailored to their unique goals, strengths, and preferences.

This is a dynamic and relationship-driven role that combines advocacy, case coordination, quality assurance, and person-centered planning. You will oversee the development and implementation of Individual Support Plans (ISPs), monitor progress toward meaningful outcomes, provide guidance and training to staff, and ensure services remain responsive to each individual's evolving needs.

This position requires travel, A valid driver’s license and reliable personal vehicle are required.

Essential Duties and Responsibilities:
  • To ensure all individuals serviced are treated with dignity and respect, thereby, facilitating agency philosophy and mission.
  • Facilitate intake meetings, document IEP meetings, team meetings, and reviews as applicable for assigned consumers.
  • Development and monitoring of individualized service goals and objectives, ensuring consistent and timely implementation of strategies to achieve those goals and all prescribed programs.
  • To provide/participate in providing and facilitation staff training
  • To complete and review all assigned documentation, including implementation plans, annual summaries, progress notes, emergency contact information etc. Maintain individual and program records and files.
  • Maintain contact with families and outside agencies. Represent and convey the Agency's Philosophy and Mission to the families, the community, funding resources, and outside agencies.
  • To develop, implement and monitor individualized program activities and community based recreational activities.
  • To develop community relationships and contacts that promote and facilitate meaningful inclusion of individuals in community based activities.
  • Ensure all assigned consumers are informed of rights in a manner consistent with their individual learning style. Protect consumer rights. Inform the Office of Quality Assurance of pertinent issues as mandated.
  • To follow, direct and implement all clinical services as prescribed.
Services Include:

Other Duties
  • To adhere to, promote and model all agency policies and to ensure all assigned staff adhere to, promote and model all agency policies.
  • Participate in regularly scheduled adult service meetings and function as part of the larger adult service team in relation to inter-departmental efforts and service development.
  • Responsible for the planning & coordination of transportation resources for scheduled recreation events.
  • Other duties as assigned.
  • Provide direct support or back-up as needed to ensure consistent implementation of services for assigned participants
  • Assist in the development and facilitation of training.
QualificationsQualifications / Requirements:
  • BA/BS Preferred. At least 3 years of experience with individuals with developmental disabilities.
  • At least one-year supervisory experience required.
  • Valid driver’s license with acceptable driving record required. Access to and provision of an insured automobile on an ongoing basis.
  • Willingness and ability to complete CPR, NCI, Human Rights, and Management training.
  • Ability to communicate effectively both in written and oral formats.
  • Ability to read, comprehend policies and instructions, and use varying degrees of independent judgment and discretion.
