Job summary

Assistance Plus is seeking a detail-oriented Shared Living Coordinator to monitor and oversee Shared Living Providers, ensuring services meet the needs of adults with developmental disabilities and/or autism. This is a full-time role based in Waterville, ME, with field visits to clients’ homes.

The coordinator will drive to client homes, maintain documentation, coordinate with Case Managers, SLPs, and Guardians, and ensure adherence to MaineCare rules and HCBS guidelines.

Qualifications

• High school diploma or equivalent required.
• Associate's degree preferred.
• A minimum of one year working in healthcare or developmental disability and/or autism fields in a supervisory/administrative level position.

Responsibilities

• Recruiting and retaining Shared Living Providers.
• Conducting home visits to assess provider suitability and safety.
• Entering data into Evergreen for all reportable events related to members.
• Maintaining regular contact with the SLP according to contract and MaineCare standards.
• Ensuring the client’s plan of care drives all service activities.
• Completing annual paperwork for client and SLPM.
