Assistance Plus is seeking a detail-oriented individual to join their team as a Shared Living Coordinator.The Shared Living Coordinator (SLC) provides the necessary monitoring and oversight of Assistance Plus Shared Living Providers (SLP). The SLC ensures services provided by the Shared Living Provider focus on the specific needs of the members who are adults with a developmental disability and/or autism. The SLC will ensure the quality and safe delivery of approved in-home and community services as outlined in the Support or Comprehensive Waivers, and in the HCBS Global rule.This is a full time (40 hour per week) position located at our Waterville office.Because the SLC will drive to a client’s home in order to monitor services, proof of vehicle insurance at $50K/$100K and a valid Maine driver’s license are required.Duties include and are not limited to:Recruiting and retaining Shared Living ProvidersEnsuring the completion of reference and background checks completed by HRParticipating with the Case Manager (CM), SLP, member, and/or Guardian in the matching processConducting home visits to assure the Provider has the skill set and lifestyle to support the individualEntering data into Evergreen for all reportable events related to membersDocumenting home visits, phone contacts, and consultationsEnsuring that the client’s plan of care drives all service activitiesMaintaining regular contact with the SLP according to contract and MaineCare standardsConducting home care compliance visits to assess compliance with health and safety codes, behavioral regulations and behavior plansAssuring the completion of progress notes and medication administration reports by the SLPConducting home visits every other month at minimum alternating with every other month phone calls, member must be present for 2 documented visits annuallyEnsuring the completion and accuracy of daily documentation per MaineCare benefits manualCompleting the Shared Living Home Visit (SLHV) tool twice a yearReporting to CM, OADS, and Guardian issues with medication administration, documentation, and significant issues impacting the member’s supportCompleting a well water test at intake and annually thereafterAttending annual PCP meetings with Case Manager, SLP, and clientCompleting all annual paperwork for client and SLPMaintaining respite care optionsCompleting weekly tracker reviewPartnering with CM to coordinate services and share informationEnsuring the execution of a contract annually with the SLP according to regulationsProviding or offering available trainings to the SLP for free if availableEnsuring the SLP has completed trainings required of the contractAttending MACSP Shared Living and CSP meetings, along with other trainings provided by OADS consistently to grow knowledge of servicesFollowing and meeting strategic planning requirementsEnsuring SL Providers comply with Home and Community Based Service (HCBS) regulationsPromoting HCBS guidelines around the promotion of competitive integrated employmentUpdating SL Operations manual as needed to meet HCBS regulations and comply with agency updatesCarrying a minimum caseload of 20 contracted Shared Living cases with expectation to continue growing the program continuouslyMaintaining and updating Shared Living Orientation Manual as changes occur in HR policiesMaintaining Workers Compensation waivers without lapse and all contractual agreementsMaintaining and updating Standard Operating Procedures as changes occurPerforming other duties as assignedPosition Requirements:High school diploma or equivalent required; Associate's degree preferredMust be 18 years of age or olderProof of vehicle insurance at $50K/$100K and a valid Maine drivers license are requiredA minimum of one year working in healthcare or developmental disability and/or autism fields in a supervisory/administrative level position.Benefits:Mileage reimbursementPaid travel time12 paid holidays per yearGenerous Earned Benefit Time401K Retirement Plan (up to a 4% employer match: 100% on the first 3% deferred, then 50% on the next 2% deferred)Employee Assistance Program (EAP)Working in a team oriented environmentAgency paid life insurance in the amount of your annual salary with a max of $50kAgency paid short term disabilityMedical, dental, and voluntary insurancesAgency issued laptop and cell phoneAssistance Plus provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
