Job summary

Living Innovations is seeking a vibrant Community Living Coordinator in Oxford, ME to support adults with IDD in shared living arrangements. You will conduct in-person visits, supervise DSPs, and collaborate with families to develop individualized support plans.

This role emphasizes advocacy, regulatory compliance, and connecting individuals with community resources to help them live safe, self-directed lives in their communities.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree preferred.
• One year of experience working with IDD population or in a similar social service or healthcare setting required.
• Valid driver's license and reliable transportation.
• Ability to pass comprehensive background checks per state and company regulations/policies.

Responsibilities

• Conduct bi-monthly in-person home visits to provide support and ensure the quality of shared living services.
• Oversee and manage Direct Support Professionals (DSPs).
• Collaborate with individuals with IDD, their families, and other professionals to develop individualized support plans.
• Advocate for individuals' needs and connect them with community resources.
• Motivate and support your DSP team to deliver exceptional care.
• Maintain accurate documentation and ensure regulatory compliance.
