Job summary

Moms In Motion / At Home Your Way is seeking a Service Facilitator (SF) Case Manager to support Virginia families. You’ll visit client homes locally, guide families through CCC+, CL & FIS, and other waiver programs, and complete care plans that truly help clients stay safe and independent.

No degree is required, but you must have at least 2 years of relevant experience, a valid driver's license, and strong communication skills.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle.
• Excellent written and spoken communication.
• Pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
