Job summary

Moms In Motion / At Home Your Way is hiring a Service Facilitator (SF) Case Manager in Virginia. You will visit client homes locally, guide families through Medicaid waiver programs, and develop plans of care and assessments that matter.

Role emphasizes compassionate communication, organization, and community impact. A valid driver’s license is required, with background checks and references. Laptop-based remote work is also part of the job.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter more.
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license and reliable vehicle.
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities
