Moms In Motion / At Home Your Way is seeking a Service Facilitator Case Manager who will visit clients in Virginia homes to support waiver programs and ensure safe, healthy, independent living. The role emphasizes heart, organization, and community impact.

The job requires two years’ related experience, no degree required, and a valid driver’s license. You’ll communicate effectively, use email/docs/spreadsheets, and handle visits in a local travel pattern.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; we care about heart and hands-on experience.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license and reliable vehicle (no client transport).
• Pass a background check and provide two professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes locally as part of daily travel.
• Serve as the go-to guide for families navigating VA waiver programs.
• Write care plans and assessments that drive real differences.
• Troubleshoot service authorizations, timesheets and related issues.
• Build lasting relationships with families based on respect and trust.
• Work remotely from your laptop/tablet when not in the field.

What You’ll Do

• Hit the road (locally!) to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.

What We’re Looking For

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
