Job summary

Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will travel locally to client homes in Virginia, guiding families through Medicaid Waiver programs and creating meaningful care plans. The role emphasizes compassionate, organized support to help clients live safe, healthy, and independent lives.

No degree is required; 2+ years of relevant experience and a valid driver’s license are important. You’ll work remotely from a laptop and build trusted relationships with families.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience are valued above formal credentials.
• Valid driver’s license and reliable vehicle.
• Able to pass a background check and provide 2 professional references.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.

What We’re Looking For

• A valid driver’s license & reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points if you’ve got Person-Centered Thinking/Planning training.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
