Job summary

Moms In Motion is seeking a dedicated Service Facilitator (SF) Case Manager to support families in Virginia. You will travel locally to client homes, guide families through Medicaid waiver programs (CCC+, CL & FIS, EPSDT), and help ensure clients live safe, healthy, and independent lives.

No degree is required; we value heart and experience. You will work from a laptop/tablet, and daily travel is expected within VA. Strong communication and organizational skills are essential.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license and a reliable vehicle.
• Ability to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes as part of daily local travel.
• Serve as the go-to guide for families navigating Medicaid waiver programs.
• Write up plans of care and assessments that make a meaningful difference.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families based on respect, patience and trust.
• Work remotely from a laptop/tablet when not visiting clients.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
