Job summary

Moore's Care Services seeks a Service Facilitator to coordinate Medicaid Home and Community-Based Waiver services, ensuring individuals remain safely in their homes. You will conduct assessments and develop plans of care, completing routine visits in line with DMAS regulations.

The role requires strong organizational, communication, and time-management skills, with the ability to work independently and travel across Northern Virginia. Training in DMAS facilitation is preferred.

Qualifications

• Some college preferred; high school diploma or GED required.
• Experience working with individuals with physical, intellectual/developmental, or mental health disabilities required.
• Completion of Virginia DMAS Service Facilitation Module Training (or willingness to complete).
• Knowledge of Virginia Medicaid Waiver programs and person-centered planning is preferred.
• Excellent documentation, computer, communication, and organizational skills.
• Valid driver's license, reliable transportation, and the ability to pass all required background screening.

Responsibilities

• Coordinate and monitor Medicaid HCBS waiver services to help individuals remain in their homes.
• Conduct assessments and develop/update person-centered Plans of Care.
• Complete routine and reassessment visits in compliance with Virginia DMAS regulations.
• Monitor individuals' health, safety, and welfare and identify changes needing additional services or interventions.
• Maintain accurate documentation and submit required reports on time.
• Collaborate with individuals, families, healthcare providers, and community resources.
• Travel throughout Northern Virginia as required.

The role requires maintaining accurate documentation, submitting required reports on time, and collaborating with members, families, healthcare providers, and community resources. Strong organizational, communication, and time-management skills are essential, as this position requires working independently and managing a flexible schedule. Reliable transportation and the ability to travel throughout Northern Virginia are required.
