Job summary

Hill Country Mental Health and Developmental Disabilities Centers seeks a Service Coordinator to assess needs, advocate for individuals, and develop Person Directed Plans. You will collaborate with service providers, deliver skills training, and maintain positive relationships to support wellbeing.

The role includes documenting accurately, scheduling meetings, ensuring compliance, and handling crisis interventions with minimal supervision.

Qualifications

  • Bachelor's or advanced degree from an accredited college or university.
  • Associate degree in social/behavioral health or related field including psychology or human services.
  • High school diploma with two years paid/unpaid experience with individuals with intellectual or developmental disabilities.
  • Minimum 1 year experience working with individuals with developmental disabilities.
  • Valid Texas driver’s license and auto insurance.
  • CPR certification and Prevention and Management of Aggressive Behavior training within 3 months.

Responsibilities

  • Assess needs and develop life management and Person Directed Plans.
  • Provide relevant skills training to improve daily life.
  • Collaborate with agencies to liaison for effective care and outcomes.
  • Write and document clearly with proper grammar for value.
  • Schedule and conduct meetings per state and company guidelines.
  • Maintain compliance with legal requirements and policies.
  • Respond to crisis intervention with minimal supervision.
  • Maintain progress meetings and protect confidentiality per HIPAA.

The Service Coordinator will assess individuals' needs, advocate for yourpeople, and develop and monitor Person Directed Plans. The ServiceCoordinator will collaborate with service providers and agencies to supporteffective Person Directed Plans. The Service Coordinator will provideskills training, crisis prevention and management, and maintain a positiverelationship with individuals, family members, and service providers to supportsuccessful, person-centered outcomes. This compassionate, caring, andsupportive team member will contribute to the overall lifetime potentialsuccess of an individual living with intellectual and/or developmentdisabilities and their state of wellbeing.

Youwill get to:
  • Assess individual needs and collaborate with themto develop and execute effective life management and Person DirectedPlans.
  • Provide relevant skills training to improve dailylife.
  • Collaborate with service providers and agencies tobe a liaison for effective care and positive outcomes.
  • Write and document articulately while usingcorrect grammar so that documentation is clear and valuable.
  • Schedule and conduct meetings that adhere to bothstate and company provided guidelines.
  • Maintain compliance with legal requirements andcompany policies and procedures.
  • Respond to crisis intervention with minimalsupervision.
  • Consistently meet with individuals to maintainprogress and compliance.
  • Guard confidentiality with knowledge and adherenceto HIPPA regulations.
You'll definitely need:
  • A bachelor's or advance degree from an accreditedcollege or university.

or

  • An associate degree in a social, behavioral humanservice, or health related field including, psychology, social work,medicine, nursing, rehabilitation, counseling, sociology, humandevelopment, gerontology, educational psychology, education, and criminaljustice.

or

  • A high school diploma or a certificate recognizedby a state as the equivalent of a high school diploma, and two years ofpaid or unpaid experience with individuals with intellectual ordevelopmental disabilities.
  • Minimum 1 year of experienceworking with individuals with developmental disabilities.
  • Maintain valid Texas state Driver's License and auto insurance.
  • Complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment

  • Provide exemplary customer service and care toindividuals with their best interest at heart.
  • Work autonomously to complete assignments withincompany and state parameters with a high level of quality.
