Job summary

Community Access in New York City seeks a Service Coordinator to provide engagement and comprehensive support within our supportive housing programs. You will use a person-centered model to help participants pursue recovery, access services, connect with community, family, and educational or employment opportunities.

Responsibilities include leading individual supports and community-building group activities, completing assessments and service plans on time, and documenting notes within required

Qualifications

• High school diploma or GED required; bachelor's degree preferred in related field.
• Excellent oral and written communication skills.
• Bilingual Spanish-speaking preferred; ability to speak other languages helpful.

Responsibilities

• Provide engagement and comprehensive support to participants in Community Access’ housing programs.
• Lead individual supports and community-building group activities; ensure goals are tracked and documented.
