Job summary

Community Access in New York City seeks a Service Coordinator to provide engagement and comprehensive support within our supported housing programs. You will work with participants to maintain housing, pursue recovery goals, and connect to community and educational opportunities.

This role emphasizes person-centered planning, group activities, and timely documentation, with a focus on building community ties and empowering residents to move forward.

Qualifications

• High school diploma or equivalent required; bachelor’s degree preferred.
• Excellent oral and written communication skills.
• Ability to prioritize and meet deadlines.
• Ability to work independently and as part of a team.
• Bilingual Spanish-speaking preferred.
• Must be fingerprinted and cleared through a background process.

Responsibilities

• Provide engagement and comprehensive support services to program participants.
• Support participants using a person-centered model to help maintain housing and achieve goals.
• Coordinate and lead group activities within the housing program.
• Complete assessments and service plans on time.
• Document notes and maintain case records within 48 hours of meetings.
• Conduct monthly in-apartment visits and tenant supports.
