Job summary

Community Access in New York City is seeking a Service Coordinator to provide engagement and comprehensive support to participants in supported housing programs. You will work with individuals to maintain housing, pursue recovery, and connect to treatment, education, and social networks using a person-centered approach.

You will organize community-building activities and ensure timely documentation, assessments, and service plans.

Qualifications

• Excellent oral and written communication skills.
• High school diploma or GED required; bachelor’s degree preferred.
• Ability to prioritize and meet deadlines.
• Be creative and flexible.
• Ability to use MS Word and Excel.
• Bilingual Spanish-speaking preferred.

Responsibilities

• Provide engagement and comprehensive support services to program participants.
• Support participants using a person-centered model to maintain housing and reach goals.
• Deliver individual support and lead community-building group activities.
• Complete assessments and service plans on time.
