Job summary

Crestwood Behavioral Health in San Luis Obispo is seeking a Service Coordinator (Mental Health Case Manager) to assess clients, coordinate treatment plans, and support discharge planning with the nursing team. This full-time role offers comprehensive benefits and growth within California’s leading behavioral health provider. Schedule: AM (Tue–Sat).

On-site work with a collaborative multidisciplinary team. Pay range: $24.20–$26.19 per hour; so on.

Qualifications

• Two years of full-time experience or part-time equivalent in a mental health setting.
• Bachelor's degree may substitute for two years of experience.
• Knowledge of DSM-5 and psychosocial rehabilitation principles.
• Familiarity with regulatory standards.

Responsibilities

• Coordinate interdisciplinary team meetings and discharge planning efforts.
• Oversee overall treatment plan implementation and collaborate with nursing staff to ensure continuity of treatment.

Title: Service Coordinator (Mental Health Case Manager)

Job Duties

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Schedule

Full-Time: AM (Tues-Sat)

Qualifications

• Two (2) years of full-time experience or part-time equivalent in a mental health setting.
• A bachelor's degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

Pay Range

$24.20 - $26.19 USD

The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.
