Job summary

Crestwood Behavioral Health, a California-based provider in San Francisco, seeks a Service Coordinator (Mental Health Case Manager) to join our compassionate team. You will assess clients, coordinate treatment plans, and support discharge planning alongside an interdisciplinary group.

Qualifications

• Two years of full-time experience in a mental health setting, or part-time equivalent.
• Bachelor’s degree may be substituted for the two years of experience.
• Knowledge of DSM-5 and psychosocial rehabilitation principles.
• Familiarity with regulatory standards.

Responsibilities

• Assess clients to identify areas needing treatment and complete assessments.
• Coordinate interdisciplinary team meetings and discharge planning.
• Oversee treatment plan implementation and collaborate with nursing staff.

Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Schedule:

• Full-Time: PM (Tues-Sat)

Qualifications:

• Two (2) years of full-time experience or part-time equivalent in a mental health setting.
• A bachelor’s degree with a major in psychology, social work or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.

Pay Range:

$26 - $28.71 USD
