Job summary

UPMC Western Behavioral Health at Safe Harbor seeks a Service Coordinator I to support Blended Case Management, coordinating services to help individuals achieve health and personal goals in an ambulatory, community-based setting.

This role involves assessment, service planning, advocacy, and connecting clients with community resources; schedule Monday–Friday 8:30am–5:00pm with on-call rotations. A sign-on bonus of $3,000 is offered.

Qualifications

• Bachelor’s degree in the fields listed (pending graduates welcome)
• Valid driver’s license
• Clearances: Act 31, Act 33, Act 34, Act 73

Responsibilities

• Coordinates mental health services, promoting recovery through advocacy and independent judgment.
• Develops and implements comprehensive service plans with individuals, families and support systems.
• Facilitates interdisciplinary meetings to ensure needs are met and services are accessible.
• Provides support connecting individuals with internal and external resources.
• Maintains accurate records, participates in training, and stays current on recovery-oriented practices.
