Highland Behavioral Health is seeking a compassionate Mental Health Coordinator to support residents in achieving their goals, coordinating services, and documenting progress.

You will work with residents, families, and providers to develop individualized service plans and respond to crises, with a focus on recovery-oriented care and community integration.

Formación

• Bachelor's degree in Social Work or related field, or 2 years mental health experience.
• Strong communication, organization, and problem-solving skills.
• Ability to coordinate multiple activities and meet deadlines.
• Basic computer skills and valid First Aid certification.

Responsabilidades

• Provide case management and support to residents
• Assist residents with daily living skills and managing psychiatric symptoms
• Coordinate medical, social, educational, and rehabilitative services
• Develop and implement individualized service plans
• Collaborate with residents, families, staff, and outside providers
• Monitor resident progress and document service outcomes
• Prepare annual assessments, service plans, and progress notes
• Provide support during potential crises and develop appropriate interventions
• Transport residents to appointments and outings as needed
• Assist with group treatment and recovery-oriented activities
