Job summary

The Massachusetts Department of Developmental Services (DDS) is recruiting for Human Services Coordinator I in Waltham. The role serves as the primary contact for individuals and families, coordinating ISP development and ensuring services meet needs through collaboration with providers and partners.

Positions include ASD, Children’s, Adult, Transition, and Self-Direction Service Coordinators. This is a full-time role with standard weekday hours and community engagement requirements.

Qualifications

• At least three years of full-time or equivalent in human services or social work.
• Bachelor's degree or higher may substitute for two years of experience.
• Travel may be required; driver's license may be required depending on assignment.

Responsibilities

• Coordinate and facilitate development of Individual Service Plans (ISP) for individuals on caseload.
• Monitor the implementation of services provided by DDS-funded residential, day and support program providers.
• Arrange and coordinate Department-funded and generic supports in response to individual needs.
• Maintain communication and outreach with community services and other state agencies.
• Support specialized roles (ASD SC, Children’s SC, Adult SC, Transition SC, Self-Direction SC) as described in the postings.
