Job summary

Qualifications

• At least three years of full-time or equivalent experience in human services work or social work.
• Bachelor's degree or higher may substitute for two years of required experience.
• Travel may be required based on assignment.
• Salary placement follows internal guidelines and bargaining agreements.

Responsibilities

• Serve as primary contact for individuals and families served by the DDS Area Office.
• Coordinate and facilitates the development of ISPs/ITPs/Life Plans for individuals on the caseload.
• Arrange and monitor DDS-funded and community supports to meet individual needs.
• Maintain communication with individuals, families, providers, and other agencies.
• Participate in specialized roles (Transition, Adult, ASD, Children, Self-Direction) as assigned.
