Job summary

The Commonwealth of Massachusetts, Department of Developmental Services, seeks a Human Services Coordinator I to manage ISP/ITP planning and coordinate services for individuals with developmental needs in Beverly, MA. Qualifications

• At least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work.
• Substitutions: a BA/BS may substitute for two years of experience.
• Travel may be required for this assignment.

Responsibilities

• Coordinate and facilitate development of ISP/Life Plan/ITP for individuals on caseload.
• Monitor service provision and arrange DDS-funded and generic supports.
• Maintain communication with individuals, families, guardians, providers and agencies.
