Job summary

Commonwealth of Massachusetts seeks a Human Services Coordinator I (HSC I) at Hogan Regional Center in Danvers, MA. The role focuses on coordinating plans for individuals and families, ensuring services meet needs across DDS-funded programs and community supports.

You will work with diverse stakeholders, support transitions for youth and adults, and participate in regular case management, visits, and documentation to ensure quality service delivery.

Qualifications

• At least three years of full-time or equivalent experience in human services work or social work.
• Bachelor's degree or higher may substitute for two years of required experience.
• Travel may be required.
• Substitutions for related fields are allowed for certain assignments.

Responsibilities

• Coordinate and facilitate development of ISP, Life Plan, or ITP for individuals on caseload.
• Monitor implementation of DDS-funded services across residential, day, and support programs.
• Maintain communication with individuals, families, guardians, provider agencies, and community partners.
• Provide crisis intervention and advocacy as needed; arrange and/or provide testing and follow-up planning.
