Job description

Service Coordinator - Early Intervention

Join to apply for the Service Coordinator - Early Intervention role at Upstate Cerebral Palsy.

**Salary:** $17.00/hour Training Rate, $11/unit

The Early Intervention Service Coordinator establishes relationships with families of children aged birth to 3 years with developmental disabilities. They create and sustain services identified through the development of an Individual Family Service Plan. Responsibilities include scheduling and conducting meetings, making referrals, and providing advocacy and education for children and their families.

Core Responsibilities
  • Provides initial and/or ongoing service coordination across Oneida County.
  • Ensures all services are established and maintained for the families.
  • Assures supports are in place, including service provider and family contacts.
  • Collaborates with all service providers and establishes team communication plans.
  • Monitors goals and the quality of team interactions.
  • Advocates for additional services and linkages as appropriate.
  • Provides written documentation of services.
Qualifications
  • Minimum of one of the following:
    • Two years’ experience in service coordination activities, verified on a pro-rata basis.
    • One year of service coordination experience plus one year in a related service setting with infants/toddlers with developmental delays or disabilities.
    • One year of service coordination experience and an Associate’s degree in a health or human service field.
    • Bachelor’s degree in a health or human service field.
    • License, certification, or registration in a relevant profession.
  • Must obtain the NYS Early Intervention Coordination Certification within the first 3 months.
  • Complete 40 hours of specialized training within the first year.
  • Valid NYS driver’s license required.
