Job summary

Talentify in Texas is seeking a Service Coordinator to assess coordination needs, plan services, refer, monitor provision, and advocate for consumers. You will document progress and maintain regular contact with families and providers.

The role requires a degree in a related field or an equivalent combination with IDD experience; bilingual Spanish/English is a plus. Salary is $19.00 per hour, 8–5, Monday through Friday.

Qualifications

• Bachelor’s degree or advanced degree in any field, or an associate degree with IDD experience.
• Bilingual in Spanish & English is a plus.

Responsibilities

• Assess and verify service coordination needs.
• Plan services, referrals, and monitor provision.
• Advocate for consumers.
• Provide after-hours crisis services.
• Document progress of interventions.
• Frequent contact with consumers, families, and service providers.
• Flexible schedule including after-hours, holidays, and weekends.

Education

Bachelor's degree or advanced degree (any field)

Associate degree in social, behavioral, human service, or health-related field with IDD experience

High School diploma with 2 years IDD experience
