Job summary

Jobtailor is seeking a dedicated case manager to support individuals with developmental disabilities and their families. You will advocate, coordinate services, and maintain comprehensive documentation while collaborating with multidisciplinary teams in a community-based setting.

Responsibilities include managing caseloads, creating action plans, and ensuring timely utilization reviews and reimbursements. Strong communication and independence are essential.

Qualifications

  • Bachelor’s degree or RN license is required.
  • Must live in the service area and have reliable transportation.
  • Driver’s license, car insurance, and internet access required.
  • Ability to collaborate with professionals from multiple disciplines.
  • Strong written and oral communication skills.

  • Advocate for, monitor, and evaluate services with individuals and families.
  • Manage a caseload of varying ages, disabilities, and case complexity.
  • Establish trusting relationships through regular contact and planning.
  • Identify needs and create action plans.
  • Develop community relationships and support systems.
  • Research resources, make referrals, and provide follow-up support.
  • Prepare Utilization Review requests and service authorizations.
  • Coordinate federal, state, county, and community programs.
  • Monitor and assess quality, timeliness, and effectiveness of services.
  • Conduct monitoring by phone, email, and in-person meetings monthly, quarterly, and annually.
  • Follow reporting protocols, including mandated reporting for abuse and neglect.
  • Complete written case-note documentation within three business days of TCM activity.
  • Maintain a minimum of 120 TCM billable hours per month.
  • Complete assigned training and participate in continuing education, conferences, and seminars.
  • Carry out special assignments or duties.

Job description

  • Help people with developmental disabilities and their families identify and obtain needed services and supports
  • Advocate for, monitor, and evaluate services with individuals, families, or guardians
  • Manage a caseload of varying ages, disabilities, and case complexity
  • Establish trusting relationships through regular contact and planning
  • Identify needs and create action plans
  • Develop community relationships and support systems
  • Research resources, develop new resources, make referrals, and provide follow-up support
  • Prepare Utilization Review requests and service authorizations
  • Coordinate federal, state, county, and community programs
  • Monitor and assess the quality, timeliness, and effectiveness of services and supports

Requirements
  • Bachelor’s degree from an accredited college or university or Registered Nurse license
  • Applicant must live in the service area
  • Reliable transportation and an acceptable driving record
  • Own vehicle for work-related transportation
  • Ability to drive to individuals’ homes for face-to-face meetings
  • Valid driver’s license
  • Proof of personal, valid vehicle insurance
  • Reliable internet for telecommuting employees
  • Ability to work with professionals from multiple disciplines
  • Effective oral and written communication
  • Ability to utilize multiple computer formats
  • Ability to work with economically and culturally diverse backgrounds
  • Ability to meet specified timelines accurately and return calls and emails within one business day
  • Ability to work independently and be resourceful

  • Ability to exercise discretion regarding confidential information
  • Ability to advocate for individuals served
  • Ability to facilitate and lead an ISP team
  • Preferred experience working with individuals with intellectual and/or developmental disabilities in a community-based setting or related fields
  • Preferred experience with case management and documentation applications such as Setworks, Kronos, and Therap
  • Preferred multidisciplinary team experience assessing, developing, or implementing person-centered plans
  • Preferred experience using Microsoft Office Suite
