Job summary

MyPath is hiring a Service Coordinator (Program Manager) to lead multiple programs, supervise staff, and serve as the main liaison between guardians, clients, and the company. The role includes field site work and administrative documentation within homecare settings for adults with behavioral or medical disabilities.

You will interview hires, develop ISP plans, ensure service delivery, and manage emergencies. Travel between sites is required; a Wisconsin driver’s license is needed.

Qualifications

  • High School Diploma or equivalent, Bachelor’s preferred.
  • 3 years experience in direct care and 2 years supervisory experience preferred.
  • Excellent communication and organizational skills.
  • Flexibility, autonomy, responsiveness, and a high level of initiative.
  • Ability to travel to different work locations, training sessions, and meetings.

Responsibilities

  • Interview and onboard new hires, including training on standard precautions.
  • Develop and manage ISP for assigned clients across programs.
  • Ensure ISP delivery for client population across programs.
  • Respond to program emergencies, including outside normal hours.
  • Maintain medical treatment supplies and train staff on administration/logging.

This role leads programs that support adult clients with behavioral and/or medical disabilities.Work with clients directly in the field and spend time in the office for documentation.
