MyPath is seeking a dedicated supervisor to oversee service delivery and ensure care plans are implemented accurately for children, adolescents, and adults with special needs.

As a company-owned by employee-owners, you will coach staff, manage schedules, ensure licensing compliance, and maintain detailed documentation to support positive client outcomes.

Responsibilities

  • Monitors service and care provisions for quality and effectiveness.
  • Ensures implementation of Individual Service and Behavior Plans as intended.
  • Reports service plan and care provision concerns to Supervisor.
  • Develops and implements changes to service plans and care provision to ensure positive outcomes.
  • Monitors health status of clients and directs concerns to the medical team.
  • Reviews physician orders and treatments as prescribed.
  • Ensures weekly meal menus are developed and posted respecting dietary needs.
  • Manages budget and shopping for groceries and supplies.
  • Completes monthly activities calendar for client engagement.
  • Assesses employee work performance and provides regular feedback and coaching.
  • Generates weekly work schedules to minimize overtime and cover shifts.
  • Conducts service location and client-specific training for staff.
  • Monitors condition of location, furnishings, equipment, and grounds; reports issues.
  • Maintains proper petty cash and client funds records as per policy.
  • Performs direct care services as documented in the DSP position description.
  • Keeps written updates professional and clear.
  • Implements new policies and forms per company standards.
  • Upholds confidentiality and professional conduct with clients and staff.
  • Presents a professional appearance and interactions with clients and management.

Job description

Ensures the implementation of Individual Service and Behavior Plans as intended. Monitors accurate and appropriate documentation related to all aspects of assigned service location and care provision per Company policy. Reports service plan and care provision concerns to Supervisor. May be required to develop and implement changes to service plan and provision of cares to ensure positive outcomes under the direction of the Supervisor.Monitors health status of clients and addresses changes in health or other issues by directing concerns to the client’s medical team. Reviews and monitors implementation of physician orders and treatments as prescribed.Ensures weekly meal menus are developed and posted that consider client preference and adhere to client specific dietary needs, requirements, and restrictions when clients are unable to do so independently. Manages budget and conducts or delegates shopping for groceries and supplies.Ensures completion of the monthly activities calendar to include opportunities for clients to engage in community and home-based recreational activities based on client preferences when clients are unable to do so independently.Assesses employee work performance. Provides ongoing and regular feedback to assigned staff. Conducts employee performance evaluations in accordance with Company standards. Provides regular coaching and corrective action as needed per HR recommendations.Monitors and positively impacts morale. Seeks regular feedback from staff and maintain an open, collaborative communication style to encourage staff engagement.Generates weekly work schedules that minimize the use of overtime and maximize staff utilization while ensuring all shifts are covered in the absence of a dedicated Scheduler. Monitors staff coverage and accurate timekeeping practices.Conducts service location and client specific training for all direct reports and for staff who cross train at assigned service location.Monitors physical condition of assigned service location, furnishings, equipment, and grounds. Report damage, problems, hazards, or repair needs in a timely manner per policy. Ensures assigned service location is clean, free from hazards, and meets Company and licensing standards at all times.Maintains cleanliness of Company owned vehicles. Facilitate regular maintenance and promptly report need for extraordinary repairs or maintenance to Coordinator.Manages proper use and record keeping of petty cash, credit card transactions, and client funds in accordance with Company and licensing standards.Performs direct care services in accordance with the Direct Support Professional (DSP) essential functions as documented in the Position Description.Produces regular written updates that are professional and easily understood.Demonstrates knowledge of organizational policies and procedures, applicable licensing standards, administrative procedures, confidentiality requirements, and Employee Handbook information. Implements new company policies, forms, and contractual standards as directed in a manner that ensures consistent practices and meets desired outcomes of the Company.Observes all rules of confidentiality both within the programs and outside of work.Presents oneself in a professional manner at all times, including appearance, language, and interactions with clients, co-workers and management team.
