The Service Coordinator (Housing) will facilitate individual service plan development and provide intensive, community-based support. This role involves accessing a full range of services to help participants attain housing stability. Responsibilities include assessing participants' strengths and needs, developing documentation, implementing goals and plans, providing referrals, and assisting with accessing support services to overcome barriers to housing. The Service Coordinator will monitor progress and modify plans as needed, playing a key role in meeting housing needs and connecting clients with community resources to improve outcomes.

External Hiring Range: $20.08 - $20.88/hour
Schedule: Monday to Friday, 8:00 a.m. - 4:00 p.m., with some evening and weekend hours as needed.
Travel: Yes, local travel required.
Degree in Human Services, Psychology, Rehabilitation Science, Social Work, or related field required.
Experience in locating community and social services for at-risk populations required. Preferred experience includes homelessness, supportive housing, mental health, D/A, unemployment, ex-offenders, and CYF.
Retention Bonus: $1,000 after 90 days of employment.
