Job summary

Upper Valley Services (UVS) in Vermont is seeking a Service Coordinator to support people with developmental disabilities and their families. This role blends field work and office duties in Moretown, focusing on coordinating services and ensuring state requirements are met.

You will work independently and as part of a team, manage documentation, meet deadlines, and participate in thorough training. A college degree in Human Services or related field is preferred, and a valid driver's license

Qualifications

Responsibilities

• Meet at least monthly with the shared living provider and participant in their home to support and manage state-required expectations for a healthy environment that meets the participant’s goals and needs
• Balance field work with main office responsibilities in Moretown
• Maintain thorough documentation and case notes
• Participate in initial and ongoing training programs

Upper Valley Services (UVS) is one of Vermont's designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.

UVS is seeking a Service Coordinator candidate who is:

• Upper Valley Services is a person-centered program.
• Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participant’s goals and needs.
• Mix of field work and main office in Moretown.
• Background checks are required
• Must have valid driver’s license and insured vehicle
