Job summary

UCS is seeking a Service Coordinator in Vermont to develop, implement, coordinate, and monitor services for individuals with intellectual or developmental disabilities. You will manage ISA processes, documentation, and coordination with a network of providers in home and community settings.

The role includes direct client visits, planning oversight, and collaboration with clinical teams to ensure services meet individual needs and regulatory requirements.

Qualifications

• High School Diploma or equivalent.
• At least one year of experience in Developmental Services, or a related bachelor's degree.
• QDDP (Qualified Developmental Disabilities Professional) preferred.

Responsibilities

• Facilitates and documents person-centered planning (ISA).
• Completes documentation timely; identifies strengths, needs, goals; interacts with clients regularly.
• Reviews ISA expectations and data collection systems for consistent implementation.
• Coordinates services with providers (physicians, specialists, therapists, psychologists).
• Conducts monthly home visits and adheres to state regulations.
