Qualifications

• Bachelor’s degree required; equivalent experience may substitute.
• Obtaining wraparound practitioner certificate required within 2 years.
• Capable of extensive travel and providing crisis intervention.

Responsibilities

• Facilitate individualized, family-driven care for youth.
• Coordinate a care plan involving the youth, family, and providers.
• Ensure comprehensive assessments are completed.
• Develop individualized Plans of Care.
• Assist families in accessing needed support and resources.
