Job summary

Teladoc Health is transforming how people access and experience healthcare. This Expert Coach role provides 1:1 and group diabetes coaching to empower self-management and improve health outcomes within member programs.

Certified Diabetes Care and Education Specialist (CDCES) credentials and ADCES 7 DSME/T knowledge are essential. Minimum 3 years in healthcare, bachelor’s degree, and strong motivational interviewing skills are required.

Qualifications

• Bachelor’s degree required.
• CDCES certification required and maintained.
• 75 CEUs in 5 years to maintain CDCES.
• Applies ADCES 7 DSME/T to practice.
• 3+ years in healthcare field.
• Empowers members to self-manage chronic conditions.
• Enthusiastic and energetic in member interactions.
• Coaching aligned with Motivational Interviewing.

Responsibilities

• Empowers members to self-manage chronic condition.
• Assess needs, barriers, and motivation to educate/coach.
• Coaching aligned with Motivational Interviewing approach.
• Attend mandated trainings and meetings as required.
• Follow scheduling to meet coaching utilization and needs.
• Participate in process improvements with a solution mindset.
• Mentor less experienced coaches and provide feedback.
• Assist with content development (handouts, videos, etc.).
• Stay updated on clinical best practices in coaching.
• Lead diabetes education sessions using ADCES 7 framework.
• Facilitate live diabetes-focused group learning events.
• Spend up to 30% of time on liaison or cross-functional work.
