Qualifications

• Must possess and maintain NBC-HWC certification.
• Up to date with CEUs to maintain NBC-HWC.
• Minimum of a bachelor’s degree.
• At least 3 years of professional experience in healthcare.
• Ability to interpret ADCES 7 DSME/T education model for diabetes care.
• Strong background knowledge of diabetes treatment options.

Responsibilities

• Provide 1:1 coaching for whole person care.
• Educate and coach to drive behavior changes improving clinical outcomes.
• Support content development (handouts, videos, podcasts, presentations).
• Stay updated on clinical best practices for coaching.
