Job summary

Centene Corporation is seeking a field-based Care Manager to assess and coordinate medical and support services for complex/high acuity populations with primary medical needs. The role develops personalized care plans, educates members and families, and liaises with providers to ensure high‑quality, cost-effective care across the continuum.

The position requires NY RN licensure (or NP) and Suffolk County residency.

Qualifications

• Requires Graduate from an Accredited School of Nursing or a Bachelor’s degree and 4–6 years of related experience.
• Bachelor’s degree in Nursing preferred.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and assesses long-term care plans and collaborates with care management to identify providers and resources.
• Coordinates between member/family and care providers to ensure person-centered care.
• Monitors and revises care plans to meet member goals and needs.
• Provides education to members and families on disease processes, services, and benefits.
• Acts as liaison between member, physician, and facilities.
