Job summary

Centene Corporation is seeking a field-based Care Manager in New York to assess and coordinate all aspects of care for complex/high acuity members with primary medical needs. The role focuses on developing individualized care plans and educating members and caregivers on available services and benefits.

Responsibilities include coordinating between members, families, and providers, monitoring progress, updating plans for best outcomes, and ensuring compliance with state and clinical guidelines.

Qualifications

• Graduate from an Accredited School of Nursing or a Bachelor’s degree with 4–6 years of related experience
• Bachelor’s degree in Nursing preferred
• RN licensure required or NP licensure as applicable

Responsibilities

• Assess and coordinate medical and supporting services for high acuity populations
• Develop personalized care/service plans for long-term care members
• Educate members and families on services and benefit options
• Coordinate between member/family and care providers to ensure person-centered care
• Monitor care plans and adjust as needed to meet goals
• Collaborate with healthcare providers to revise care plans
• Collect and maintain member information ensuring regulatory compliance
• Provide education on disease processes, care gaps, and care options
• Act as liaison between member, family, physician, and facilities
