Job summary

Requires NY RN licensure (or NP), graduate nursing education, and 4–6 years of related experience. Travel within Suffolk County is expected and the role supports a flexible schedule.

Qualifications

• Graduate from an Accredited School of Nursing or a Bachelor's degree and 4–6 years of related experience.
• RN or NP licensure required (State Licensure and/or Compact State Licensure).
• New York residency and RN licensure required for eligibility.

Responsibilities

• Assess and coordinate medical and supporting services across the continuum of care.
• Develop personalized care/service plans for long-term care members and educate families/caregivers.
• Coordinate with care teams, monitor progress, adjust plans, and ensure compliant, person-centered care.
