Job summary

Centene seeks a Senior UAS LTSS Assessor (RN) to perform care management duties, assess and coordinate medical services for complex populations, develop personalized care plans, and educate members and families on available services. This full-time role is remote in New York.

You will evaluate service needs, develop long-term plans with the care management team, coordinate with providers, monitor plans, identify trends, and ensure compliance with guidelines while supporting member outcomes.

Qualifications

• Graduate from an Accredited School of Nursing or Bachelor's degree preferred.
• 4–6 years of related experience.
• RN licensure required, or NP licensure as alternative.

Responsibilities

• Evaluate service needs of complex or high-risk members and recommend plans for optimal outcomes.
• Develop and assess ongoing long-term care plans in collaboration with the care management team.
• Coordinate care between members, families, and providers to ensure person-centered services.
• Monitor care plans and member status, revising as necessary to meet goals.
• Review member data to identify trends and improve care quality.
• Collaborate with healthcare providers to facilitate member services and treatments.
• Collect and maintain member information to ensure compliance with guidelines.
• Educate members and families on disease processes and healthcare options.
• Act as a liaison between members, families, and healthcare providers.
• Provide coordination of service authorization based on assessments.
