Job summary

Centene seeks a Senior LTSS Assessor (RN) to perform care management duties, assess complex member needs, develop personalized care plans, and educate families on available services.

In this remote NY role, you will collaborate with multidisciplinary teams, monitor plans, and coordinate service delivery while ensuring compliance and high-quality outcomes for high-risk populations.

Qualifications

• RN or NP licensure with State license required.
• 4–6 years of related experience in LTSS or care management.
• Bachelor's degree in Nursing or equivalent; BSN preferred.

Responsibilities

• Evaluate the service needs of complex or high‑risk members and recommend a plan for the best outcome.
• Develop and continuously assess ongoing long‑term care plans and collaborate with the care management team.
• Coordinate and manage communication between members, families, and care providers.
• Monitor care plans and member status, revising as necessary to meet goals.
• Monitor member status for complications and clinical symptoms.
• Review member data to identify trends and improve quality care.
• Review referrals and intake assessments to develop care plans.
• Collaborate with healthcare providers to facilitate member services.
• Collect, document, and maintain member information for compliance.
• Educate long-term care members and their families on healthcare options and benefits.

About This Role

The UAS Senior LTSS Assessor (RN) performs care management duties to assess and coordinate medical services for complex populations, developing personalized care plans and educating members and their families on available services.

Highlights

• Pay: $36 - $65/hour
• Location: Remote, NY
• Residency: Suffolk county residency required
• Schedule: Full Time, Day shift

Requirements

Education:

• Graduate from an Accredited School of Nursing or a Bachelor's degree required
• Bachelor's degree in Nursing preferred

Experience:

• 4–6 years of related experience required

Licenses & Certifications:

• RN - Registered Nurse - State Licensure and/or Compact State Licensure required, OR
• NP - Nurse Practitioner - Current State's Nurse Licensure required
