Job summary
Centene Management Company LLC in New York is seeking a Field-based Care Manager to assess and coordinate medical and support services for complex high-acuity populations. You will develop personalized care plans for long-term care members, educate families, and connect them with appropriate benefits and resources.
The role requires an RN license in NY (or compact) and 4–6 years of related experience, with Suffolk County residency preferred.
Qualifications
- Requires Nursing degree or equivalent with 4–6 years of related experience.
- Bachelor's in Nursing preferred.
- Licensed RN in NY required or compact state license.
- Experience in care management and long-term care services.
Responsibilities
- Develops personalized care/service plans for long-term care members.
- Educates members and families on services and benefits.
- Coordinates with care providers to address needs and resources.
- Monitors plans and revises as needed to meet goals.
- Acts as liaison between member, family, and facilities.
- Complies with state and federal guidelines.
