Job summary

Seven Counties Services is seeking a dedicated case management professional to support adults in need through comprehensive assessments, service planning, and coordinated care. You will arrange services, monitor progress, and advocate on behalf of clients to access entitlements and resources.

The role emphasizes collaboration with supervisors, providers, and families, along with timely documentation and crisis response when needed.

Qualifications

• Bachelor's degree in social work, psychology, sociology, human services, or related field.
• Completion of state-required case management/service coordination course within six months of date of employment.
• At least 1 year of post-bachelor’s experience working with adults in a human service setting; Master's degree may be considered.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client's needs.
• Participates in development of client's service plan, consistent with assessment.
• Arranges for delivery of needed services, as identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client's progress by referrals, tracking appointments, and follow-ups.
• Advocates for clients (e.g., letters to expedite entitlements).
• Prepares and maintains case records per standards.
• Provides case consultation with service providers/collaterals.
• Communicates with supervisor about case management issues.
• Provides psychosocial education to clients/families.
• Performs crisis intervention and emergency referrals.
• May serve as Primary Case Coordinator depending on skill.
