Job summary

Seven Counties seeks a qualified professional to conduct comprehensive client assessments, develop service plans, and arrange necessary services. The role includes advocacy, case coordination, and psychosocial education for clients and families.

Responsibilities cover documentation, referrals, progress monitoring, and collaboration with supervisors and providers to ensure seamless care delivery. Full-time, on-site positions with potential PCC duties based on experience.

Qualifications

• Bachelor's degree in a social work or related field as approved by Medicaid or funding source.
• Completion of state-required case management/course within six months of employment.

Responsibilities

• Conducts and documents comprehensive assessment of client needs.
• Participates in development of client's service plan.
• Arranges delivery of needed services identified in assessment.
• Assists client in accessing needed services via referrals or linkages.
• Monitors progress through referrals, appointments, follow-ups, and reassessments.
• Performs advocacy activities for the client, e.g., letters to expedite payments.
• Prepares and maintains case records per standards.
• Provides case consultation with service providers/collaterals.
• Communicates with supervisor regarding case management issues and resources.
• Provides psychosocial education to clients and family members.
• Performs crisis intervention and coordinates emergency services.
• May serve as Primary Case Coordinator (PCC) depending on skill.
